A Case of Myopathy induced by Clevudine

Abstract
Clevudine is antiviral agent for chronic hepatic B and Ebstein-Barr virus. Clevudine has a mitochondrial cytotoxicity, therefore a clevudine is able to induce myopathy associated with mitochondrial cytotoxicity. 32 years old woman has experienced muscle weakness in the lower extremity, especially in the proximal part, from 6 months ago. She has been took a clevudine 30 mg daily during 18 months. We examined serum creatine phosphokinase, lactate dehydrogenase, electromyography and muscle biopsy at the vastus medialis muscle. Serum creatine phosphokinase, lactate dehydrogenase was increased very higly and electromyography suggested the possibility of myopathic process such as small amplitude, short duration polyphasic motor unit action potential and early recruitment pattern. Scattered necrosis, degeneration and regeneration of myofibers was seen in muscle biopsy. We suspected the possibility of myopathy induced clevudine. Myopathy induced clevidine is newly appeared recently.
